Welcome to Modesto City Schools. The following information is needed for enrollment.

1. Proof of Age. At the time of application, the parent or guardian of the child shall present proof of age before the child’s application may be accepted. Such proof must be presented in the form of a certified copy of a birth record (certificate); or a statement by the local registrar or county recorder certifying the date of birth; or a baptismal certificate duly attested; or a passport; or affidavit of the parent, guardian, or custodian of the minor (Ed. Code 48002).
   
   
2. Immunization Records. See the guidelines posted by the California Department of Health Service:

3. Proof of address: See Residence Verification Form for approved items.
   
   
4. Withdrawal grades or current report card from previous school. a. Please note if GPA is less than 2.0 or student is a 20-day transfer they will be placed on the Ineligible List.
   
   
5. STAR Achievement Test Scores. (Essential for accurate academic placement).
   
   
6. CELDT Test Scores for English Learners.
   
   
7. Current I.E.P. for students receiving Special Education Services.
   
   
8. A parent must accompany the student to the office on the first day of attendance.
